VISION -- to eliminate diabetes and its complications. Diabetes Management (DM) – 6-hours of instruction, learning the basics of diabetes, treatment, complications, and the management of diabetes, portion control in meal management, the role of exercise, and blood glucose monitoring. Meal Planning - A unique meal plan designed for you according to your age, height, weight, gender, eating habits, and caloric requirements. Support Groups - English or Spanish diabetes support group: come and meet others with diabetes, share concerns, information, and recipes. Meets every month. Call for dates and times. Prevention program for children (Caring for the Future) - 4 week (6 hour) class to teach 6th, 7th and 8th grade the importance of exercise and having a healthy weight in order to prevent diabetes and other "adult onset" diseases. Outreach programs - 4 week (8 hour) class throughout the community, focusing on the importance of preventing and/or managing diabetes through healthy eating, exercise, blood glucose monitoring, and medication. Diabetes Education and Empowerment Program (DEEP) - Intensive diabetes management course. Healthy Living - Comprehensive diabetes class: includes a support session, introductory education, exercise, and a live cooking session.
Health Fairs - Blood glucose screenings, blood pressure screenings, lipid panel, and follow up/referral for abnormal results. Trial Net - Research to prevent diabetes in relatives of individuals with Type 1 diabetes. Presentations - Available for schools, churches, clubs, organizations, and businesses. Offered in English or Spanish. Walk-in screenings - Blood glucose, lipid panel, blood pressure, and Hemoglobin A1c. Some tests require fasting. Please call for requirements. Camp Lydia Mann - A week long camp full of fun activities and learning for children ages 5-12 with or at-risk for diabetes. Participating children become more independent with their diabetes care at the conclusion of camp. Camp is held the last week in June every year. Walk for Diabetes - A 2.2-mile fun walk to emphasize the importance of exercise in the control of diabetes. Walk includes entertainment every quarter mile. Gala Dinner & Dance - This is a fundraising event held in the fall to honor a person of vision in diabetes. Content Coming Soon Diabetes Expo - An exhibit of diabetes information, supplies and products; screening of glucose, cholesterol, blood pressure, eyes, feet, etc. available. Services for seniors - Scholarships for classes and outreach. Services for vision - impaired diabetics- Basic Diabetes Management and training on using adaptive equipment such as voice meter, voice thermometer, voice blood pressure cuff, and count a dose for insulin injection. Gestational diabetes - Women diagnosed with gestational diabetes are given basic diabetes management and a special meal plan. They are encouraged to keep a healthy weight after baby is born to lessen probability of developing diabetes in the future. Content Coming Soon Insulin injection training - Participants learn to draw and inject insulin, the different injection sites, insulin effects on the body, and the importance of knowing when and how much to administer. Insulin pump training - Participants switching over to the insulin pump learn the mechanics of the pump and its management. Weight Management - Three 2-hour sessions including a meal plan, exercise, and healthy eating tips to help participants get weight under control and live a healthier life in order to prevent or delay diabetes and other diseases brought on by overweight and obesity. Acanthosis Nigricans (AN) screening - Available to screen for children with signs of pre-diabetes. Diabetes Exchange Newsletter - Seasonal newsletter to let the community know the latest news in diabetes, share healthy recipes, read testimonials from clients, and learn upcoming events. Unlicensed Diabetes Care Assistants (UDCA) - School personnel learn what to look for in children with diabetes experiencing high or low blood sugars and what to do about it. They are trained on insulin injection, glucagon administration, testing methods, and what to do in the case of an emergency. Corporate Wellness - Encourages sedentary employees to make goals for exercise and healthy eating. Follow up periodically to check glucose, blood pressure, cholesterol levels, and track progress. |
